LVA Summer Camp Teacher Contract and Expectations Form
The below form serves as acceptance of a contract to teach a 2025 LVA Summer Art Camp for Louisville Visual Art, Inc. (LVA). Acceptance of the terms and conditions are outlined in the Teacher Expectations, Policies & Procedures listed below. Please make a copy of this contract letter for your records.
If you are a new camp teacher, please send the W9 form, and the Central Check Registry form along with a copy of your driver’s license.
Teachers Expectations, Policies, & Procedures
Faculty
Teachers for 2025 LVA Spring and Summer Art Camps must be either a professional artist and/or a certified art educator. Each may be observed on-site at least one time during the semester by a member of the LVA Education Committee or an LVA staff educator. All Teachers in the LVA Spring and Summer Camp program will be independent contractors who are engaged on a week-by-week basis, and LVA reserves the right to cancel any camp prior to the start of the camp for any reason.
To be eligible to teach LVA Spring and Summer Camps, CFAC, or LVA’s Open Doors program, teachers must consent to a mandatory confidential background check conducted by the Kentucky Cabinet for Health and Family Services or any other agency LVA determines is appropriate. The LVA reserves the right to update this records check at any time without notice to the individual, so long as the individual is a contract employees teaching in either the LVA Spring and Summer Camp, CFAC program, or Open Doors. All records will be kept confidential. By completing the following form you are consenting to this background check.
Compensation
Spring and Summer Camp Teachers will be paid at a rate of $675 for this week (15 hours of teaching time) plus up to 5 hours of prep time ($18 per hour). If you are teaching a shorter session Teachers will be paid $45 per hour.
Expectations of Teachers
Teachers will be expected to:
Attend the required teacher’s meeting, May 10, 10am to 11am at the United Crescent Hill Ministries building, 150 State St., Louisville, KY 40206, room 201, or make other arrangements to meet with Summer Camp Coordinators.
If you have not done so already, provide a resume.
Provide a head shot, a description about the class/teaching method, due asap. Specifications for the head shot: color photo, 300 dpi, JPEG, at least 5” x 5”. Please email to Annette at annette@louisvillevisualart.org.
Be an enthusiastic ambassador for Louisville Visual Art.
Prepare campers and art show at the end of the camp week.
Policies & Procedures
Registration: Only properly registered students are allowed to attend LVA Spring and Summer Art Camps. You will be notified of registered students by email by start of classes. If a non-registered student tries to attend class, please ask the child to leave and request that the parents contact Annette.
Attendance: Please email Annette after your first class if registered students do not attend so that she may add students if there is a waiting list for your site.
Discipline: Behavior problems are not usually a concern and are unacceptable. If problems do arise, please first talk with the student; next step if unimproved is for the teacher to contact the parent to attempt to resolve the issue. Please use email for this so that we have a record— you will need to copy annette@louisvillevisualart.org on the email so we are aware. If that is not successful, you should notify Annette.
Payment: Please send an invoice at the completion of your camp to Annette at annette@louisvillevisualart.org. An invoice should include: Your name, the name you want on the check, your mailing address, your camp name and date, your total due.
If you are new to LVA or have a name or address change please fill out a new W9 form. If you are new to LVA you must fill out a Central Registry Check form (this form requires a copy of your driver’s license) - you may email all of this information back to: annette@louisvillevisualart.org
